Phoenix Police Investigating Traffic Accident That Left Man Dead, 2 Injured

Phoenix Police are investigating a traffic collision that left one man dead and two others in the hospital.

According to the Phoenix Police Department, officers responded to a call of a serious vehicle collision at 19th Avenue and Olney Avenue. When officers arrived, they found a blue 2 door passenger car and a white SUV. Officers provided aid to the occupants of both vehicles. The fire department responded and transported two men and a woman to the hospital.

The driver of the blue vehicle was uninjured and remained on scene.

One of the men transported to the hospital, 58-year-old Forrest Wade, later died from his injuries at the hospital. The other man and woman at the hospital are expected to survive their injuries. Detectives responded to the scene and learned the blue vehicle was heading northbound on 19 Avenue at a high rate of speed when it crashed with the white vehicle that was heading westbound on Olney Avenue.

The driver was released and showed no signs of intoxication.

The investigation remains active as detectives review the evidence. A charging decision will be made at the conclusion of the investigation.
